What are the health report and the vaccination document?

The health report is a doctor/hospital document showing the person’s suitability for a particular procedure (general health situation, say). The vaccination card contains the record of the vaccinations given; it can be obtained via e-Nabız too.

What is it needed for in an Italian application?

It can be asked for in some university enrolments, in residence and health-insurance procedures, or in the health condition of particular programmes. In child/family procedures, the vaccination schedule can be needed for the school/health records in Italy.

Where and how do you get the document?

Obtain the report from an authorised hospital/doctor, and the vaccination record via e-Nabız or the family health centre. The apostille on public health documents can be taken through a Governorate/Provincial Health Directorate-linked process.
