What are a consent form and an undertaking?
A consent form shows that a person (most often a parent) gives consent to a procedure; an undertaking shows that an obligation is assumed — both notarised documents. A minor student’s travel, or a relative assuming the costs, is declared with these documents.
What is it needed for in an Italian application?
It is asked for as parental consent in the travel/education permission of students under 18, as a sponsorship undertaking that the costs will be met on a student visa, and as a declaration of the assumed obligation in some residence procedures.
Where and how do you get the document?
Have the document drawn up at a notary; the apostille is taken through the relevant District/Provincial Governorate from the notarised original. It matters that the consent/undertaking text contains the elements the Italian authority looks for (names, date, scope).

What to watch in the sworn translation
